Client Agreement

​I agree to receive treatment from Coached in Health. I am aware of the benefits and risks of my treatment and give my consent to the practitioner. I understand that there is no implied or stated guarantee of success or the effectiveness of individual techniques or series of appointments. I acknowledge that manual therapy is not a substitute for other forms of medical care, medical examination, or diagnosis. I have stated all medical conditions that I am aware of and will inform my practitioner of any changes in my health status. I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment. Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage. I understand that it is up to me to book my appointments. I understand that the staff of Coached in Health reserve the right to end the session and charge 100% of the service if they feel threatened or unsafe. I have read and understand all policies and agree to accept any cancellation fees.

Arrival for Your MLD

For all appointments, arrive 10-15 minutes prior to the scheduled starting time. This allows for time to go over the Client Intake Form, set up, carry out physical assessments, answer any questions, and allow you to change and prepare for your treatment. Early arrival allows for a relaxed and unhurried experience. If late arrival is inevitable, your massage therapist will contact you to adjust the timing or reschedule. If a client is running late, no time will be added to your time and full amount will be charged.

Clients' Needs and Boundaries

The client may choose to: leave on as much clothing as needed for comfort, refuse any modalities, stop the session at any time, and is free to leave whenever they choose.  The client will always be modestly draped. Only the area being worked on will be undraped. The clients will be kept informed of the area to be addressed. Top MLD involves the breast area, please inform your practitioner if you are uncomfortable.  Occasionally, an emotional response occurs, please feel free to express your feelings. Coached in Health offers in a safe, nonjudgmental environment or privacy and end the session.

Payment Policy

Full payment is due prior to receiving service. Payments can be made in the form of all valid cards. We do not accept Cash App, Zelle, or any other transfer apps. Please see cancellation policy for information about refunds and exchanges. Packages are non-refundable after purchase, no exceptions. Canceling a package appointment less than 24hr in advance or no showing without notice will result in the loss of a package session. Package terms and conditions are subject to change at the discretion of the business.

Insurance

Coached in Health is happy to issue receipts and SOAP notes for you to submit to your insurance company for reimbursement. Ask your insurance provider if they cover "manual therapy" or "lymphatic drainage". Our office will provide insurance billing forms so that you may be reimbursed by your insurance provider for MLD services.

Cancellation Policy

​​We understand that unanticipated events occur in everyone's life. In consideration of our clients and our commitment to providing an outstanding experience, we have adopted the following policies:

You will receive several appointment reminders via text and email prior to your appointment. Canceling or rescheduling an appointment less than 24 hours in advance will result in a penalty of 100% of the total service cost, no exceptions. No-show appointments are 100% of the total service cost. To avoid a cancellation fee, you are able to cancel or reschedule your appointment online at least 24 hours in advance. A refund will be issued and will reflect on your statement within 2-7 business days. You can also request that the charge be exchanged for use on a future appointment. If you cancel on multiple occasions or are a no-call no-show, your client status may be reassessed and/or terminated.

We reserve the right to charge 100% in the case that we can't reach you or you are unavailable when we arrive. We will wait up to 10 minutes after the designated start time. Please be advised that our staff also reserves the right to leave the session, charge the full cost of service, and ban you as a client if we feel threatened or unsafe at your location. Sexual misconduct WILL NOT BE TOLERATED.

New and Existing Medical Conditions

It is the responsibility of the client to keep the therapist informed of any medical treatment currently being taken and to provide written permission from their primary physician, chiropractor, physical therapist, etc., that the treatment may be continued.  The client must also keep the therapist informed of any changes in health conditions.  For clients undergoing chemo and radiation therapies – If you are currently in treatment, or if your last treatment session was less than 12 months ago, we require a doctor's note that states the doctor is aware of and agrees to the desired treatment. Manual Lymphatic Drainage is not available to those with the following conditions:

Decompensated cardiac insufficiency

Untreated Congestive heart failure (CHF - cardiac edema)

Acute inflammation caused by pathogenic germs (bacteria, fungi, viruses). The germs could be spread by the manual lymph drainage, with resulting blood poisoning (sepsis).

Acute renal failure

Acute deep venous thrombosis (DVT)
